‘The wish list’: Biden makes his move with push for far-reaching infrastructure overhaul

In a speech Wednesday, President Joe Biden will lay out the first part of a massive, multitrillion-dollar infrastructure plan that is expected to include projects as varied as highways and “human infrastructure,” like child care.

The kitchen-sink approach is designed to push the economy in a greener and more equitable direction, paid for with higher taxes on corporations and the wealthy, NBC News’ Alex Seitz-Wald and Shannon Pettypiece report.

The $2 trillion proposal, which is being called the American Jobs Plan, will focus on projects that could get bipartisan support, like roads and bridges, expanded broadband internet access and investments in clean energy. It also includes an expansion of home health care services, which are in demand for the aging population.

The sweeping legislative package would be legacy-making for the nascent Biden administration, but it will face a steep climb up Capitol Hill, where time to pass major bills is already waning.

“There is a sense of spending fatigue on some quarters on Capitol Hill, not only among Republicans but some Democrats, as well,” said one Democratic political strategist.

Others might argue that now is the time to act and spend his political capital: Biden currently has high national approval ratings on his handling of the economy and Covid-19, according to new polling. But, there are warning signs, too: He’s getting lower marks on culturally divisive issues like immigration and gun control.

Pfizer says its Covid vaccine is 100 percent effective in young teens

There is positive news on the vaccine front: the Pfizer and Moderna Covid-19 vaccines have been found to be 90 percent effective in their “real world” study, according to the Centers for Disease Control and Prevention. Brain Scraper: Why Do Some COVID Tests Hurt So Much?

March 30, 2021 — The one time I got tested for COVID-19, I wound up in the emergency room — but not because I tested positive. During the test, as the technician kept inching the swab deeper and deeper inside my nose, I felt a bit of discomfort. Afterward, I left the facility with a shrug, thinking it wasn’t so bad. I didn’t suspect it would trigger the worst headache I’ve ever had. I’ve had migraines for years, so I know from headaches. This felt exponentially worse, like a terrifying vise. Hours later, after over-the-counter pain medications didn’t even take the edge off, I let my husband call an ambulance.

I’m far from alone in having a COVID test with an ugly aftermath. Hattey Lennerman, a nurse in Lexington, KY, has to get monthly tests because of her work. At her very first one back in April 2020, “I felt a sharp pain and my eyes watered. I wound up gripping the chair, reeling back from the pain,” she says. She had a burning sensation and pain in her jaw. Within 15 minutes, she had the worst headache she’d had in years. “I had to turn out the lights in my office and just sit there. Ibuprofen didn’t touch it.” And at her second test, Lennerman’s nose bled.

Aside from headaches, teary eyes, and nosebleeds, some people have fainted. Others describe the test as feeling “like I was cleaned from behind my eyeball,” “wild painful,” and as if it “reached to the back of my skull and then went back for more.” One person’s eye started twitching, and another said she thought she was being lobotomized.

The Nasopharyngeal Swab

Hundreds of millions of COVID tests have been done in the U.S. over the last year. Tests using the nasopharyngeal swab — the one that feels like it might scrape your brain — are considered the gold standard.

“Think about the millions who’ve been swabbed — you hear about every one that has a bad experience,” says Andrew Lane, MD, director of the Johns Hopkins Sinus Center. “It’s extremely, extremely rare.”

Those bad experiences can stem from several issues, thanks to the number of things involved. People tolerate pain differently and have different perceptions of stimuli. And human anatomy varies widely — your nasal passages may be broader or narrower than the person standing next to you, or you may have a deviated septum. Another factor: the skill of the technician doing the test. Because testing ramped up so quickly, you’ll find a range of training and experience levels.

The premise of the test itself is another part of the problem. “The body doesn’t like foreign objects entering the nose. It’s an open path from the outside world that goes directly to your windpipe and your lungs. And your nasal cavity is adjacent to your eye and your brain,” Lane says. “That’s a high-priority territory to defend.”

To do the test, a technician inserts a flexible, soft-tipped, 6-inch swab into your nostril. They’ll guide it to the back of your nose until it reaches the nasopharynx — the area where your nasal cavities meet your throat — and swirl it gently. They may leave it there for a few seconds to collect secretions. If the first nostril doesn’t provide enough, they may repeat the process in the other nostril.

Explaining Those Reactions

The challenge, according to Lane, is that no two noses are the same. “The goal with the swab should be to get to the nasopharynx without hitting anything along the way,” he says. “Unfortunately, the geometry of the nasal passages is different from person to person.”

The mucous membrane that lines your nose has a lot of nerve endings. “In general, the body’s pretty tolerant the first couple centimeters — as far as you can stick your finger in. Beyond that, the mucous membrane reacts to being touched.” Lane says.

That activates involuntary, hard-wired reflexes. Depending on how strongly your body responds, the result can be discomfort, teary eyes, pain, and other reactions. “Some people get gagging, coughing, sneezing — it’s all part of the same reflex,” he says. For people who are more prone to get headaches, it can trigger a whopper.

As for nosebleeds, that membrane (called the mucosa) is delicate and has lots of blood vessels, says Philip Chen, MD, an associate professor of otolaryngology — head and neck surgery at the University of Texas Long School of Medicine. “Nosebleeds would occur if the swab irritated and broke the surface of the mucosa,” he said in an email. “This might be more likely in really dry climates, at altitude, and if the person has a very narrow nose.”

Fainting has an equally straightforward explanation: “It’s what’s called a vasovagal event. This can occur whenever the body has a very strong response to a situation such as emotional distress or pain,” says Chen. “For some people, the anxiety of having the procedure or the actual pain can result in fainting.”

Scraping the Brain?

Although some say the test feels like having your brain tickled or stabbed, the swab can’t actually get near your grey matter.

“There are three layers of protection in the nose. There’s the mucosal lining, which covers the inside of the nose. There’s the olfactory epithelium (involved in sense of smell). The inside, the dura mater, which means ‘tough mother,’ is a tough lining of skin around the brain. It’s hard to penetrate through (it) without something sharp,” Shawn Nasseri, MD, an ear, nose, and throat surgeon in Los Angeles, told USA Today.

In a very small number of cases around the world, testing has led to leaking cerebrospinal fluid. At least one of those was found to be due to a previously unknown birth defect. “I suspect the people who have this happen had an abnormality, a protrusion of brain tissue through a hole in the bone,” says Lane. The swab itself is flexible, so it’ll bend before getting into bone.

“It’s such a low risk, I wouldn’t be frightened,” he says. “Your risk of having COVID and not knowing is much higher.”

There is one group that may be at a higher risk for this kind of trouble: “People who have had extensive sinus surgery should not be tested with nasopharyngeal swab,” says Chen. He’s the lead author of a study that looked at the need for warning such patients. That kind of surgery often involves removing a bone between the nasal cavity and the base of the skull, which might allow the swab to get through.

Tips for a Pain-Free Swab

Because anatomy and pain tolerance vary, there’s no way of knowing if you’re likely to have a bad experience. But these suggestions may lessen your discomfort:

  • If you’ve got a stuffy nose, try a spray decongestant like Afrin, Lane suggests. That can help clear a path and make it less likely that the swab will bump into anything on its way to your nasopharynx. Lie down and put one drop in each nostril, 30-60 minutes before the test.
  • Pay attention to the technician’s instructions. Position your head as directed, so they can place the swab at the correct angle. And once they start, don’t move! “Wherever they tell you to put your head, put your head in that spot and keep it there,” says Lane.

COVID forces India’s former Gulf workers to forge new futures

It is not yet dawn but Yeroor village is long awake, the hum of productivity floating over “Gulf Street”, a lush green boulevard named for the thousands of workers who leave the southern Indian state of Kerala every year for jobs in the Middle East.

But now the workers are back, from machine operator Sudheesh Kumar, who has been forced back into manual labour in Yeroor to make ends meet, to former banker Binoj Kuttappan, who has taken up dog breeding in the state capital, Thiruvananthapuram.

In the single biggest reverse migration in more than 50 years, workers from the Gulf have streamed back to the coastal state of Kerala in the past year, propelled by a pandemic that deflated dreams of overseas riches and changing family fortunes.

While once they came home wealthy and revered, bearing gold, sunglasses, clothes and funds to buy homes, now they have returned sheepish and penniless.

“Prior to COVID, they were celebrated as heroes. Now they have nothing,” said Irudaya Rajan, a professor at the Centre for Development Studies in Kerala, who has studied migration patterns in India’s southernmost state.

“This is the first time they have returned empty-handed and will end up borrowing and selling assets,” said Rajan.

Among the Indian states, Kerala sends the most workers to the Gulf, accounting for about 2.5 million of six million Indian nationals there.

The state received about 19 percent of $78.6bn transferred to India from overseas workers in 2018, the highest state tally in the country that is the world’s top recipient of remittances.

But more than 1.1 million people have returned in the last 10 months, 70 percent having lost their jobs as domestic workers, builders, waiters, chefs and more, official data shows.

This has wrecked workers and their families’ lives and destroyed businesses dependent on the India-Gulf migration.

Kumar, 50, spent 22 years in the Middle East, with his final job in Saudi Arabia operating machines at Jeddah airport’s wastewater treatment where he earned triple the average Kerala wage.

In March 2020, he flew home – briefly, he thought – but flights were grounded in a bid to contain the new coronavirus.

Now, the father of two splits his time between farm labour and working in a stone quarry in the village of 13,000 people.

“I had planned my life when I left 22 years ago. I had any ordinary man’s dreams – a house, good education for my children,” a deflated Kumar told the Thomson Reuters Foundation outside his house, sweat beading his brow.

Kumar has been forced to sell his car and farmland to pay off a loan for his four-bedroom home in Gulf Street.

Now he is earning 400 Indian rupees ($5.50) a day compared with a fixed monthly 20,000 rupees  ($272) in Jeddah with overtime on top.

“I have no shame in doing hard labour, but how did I land here? Where did I go wrong?” Kumar said.

During the Gulf War 30 years ago and the 2008 financial crisis, many workers were forced back to Kerala, but this time the numbers are far higher number and the job market tighter.

A nationwide initiative linking returnees with jobs has notched up more than 30,000 registrants, about 80 percent of them from the Gulf States of Bahrain, Kuwait, Qatar, Saudi Arabia, United Arab Emirates and Oman, according to a government release.

Shamna Khan, 30, whose right leg is badly swollen by lymphoedema, never needed to work because her husband Shafir sent enough money home from his job in a glitzy Qatar mall.

The couple turned their mud and clay house to concrete, laid tiles, built an indoor toilet and got help for Shamna’s leg.

But after Shafir returned jobless last March, Shamna registered for India’s rural job scheme for about 300 rupees ($4) a day that guarantees a minimum 100 days of work in their village such as building roads, digging wells and trenches at farms.

“I am happy to work as I can support my family, but my leg is prone to infections,” said Shamna as she dug a trench at the village’s rubber plantation.

Sharif, who works at the quarry, worries about the looming uncertainty, his unpaid loans – and Shamna’s health.

“There is no other work here,” Sharif said.

More than 90 percent of Indian migrant workers, most of whom are low and semi-skilled workers, work in the Gulf region and southeast Asia, according to the United Nations.

Connecting them with jobs are recruitment agencies and travel firms which match workers with employers and book them on flights – a hectic business that Ajimon Mak, 45, nurtured for 14 years in Thiruvananthapuram.

“Ticketing was my main business, it was a passion and I was always busy. During the lockdown I saw it all go down to zero,” said Mak, who moved into fishmongering and recently opened a fish shop in Thiruvananthapuram.

Former banker Binoj Kuttappan, 40, also forged a new path after returning from Abu Dhabi last year following layoffs at his financial service company and decided to turn his passion for dogs into a breeding business.

“I would have never done this if not for the pandemic,” said Kuttappan, showing off seven dogs he bought for 150,000 rupees ($2,044).

With plans for a pet accessories shop, a garden for dogs and air-conditioned kennels, he has no plans to return to the Gulf – but others are counting the days until they can go back.

Kumar has started calling agencies seeking work in the Gulf.

“My savings for our future are gone and now our future looks bleak,” said Kumar. “I no longer think of making a profit. I only think of surviving the day.”

Covid cases are rising again. Who's getting sick?

People under 60 are accounting for the majority of new Covid-19 cases across the country — likely a testament to the success of the vaccines that have been administered to primarily older, more vulnerable Americans.

The number of cases is rising again following a steep decline and then plateauing for several weeks. During a White House Covid-19 briefing Monday, Centers for Disease Control and Prevention director Dr. Rochelle Walensky said the rise in cases — an average increase of 10 percent from the previous week — gave her a sense of “impending doom.”

But doctors say that patients seeking care this time around are younger and, notably, not as sick.

In New York, where cases are rising, some Covid-19 patients still require hospitalization, but the numbers of such patients are nowhere near the “astronomical” levels from a year ago, said Dr. Frederick Davis, an associate chair of emergency medicine at Northwell Health’s Long Island Jewish Hospital.

Now, Davis said, Covid-19 cases are mostly mild.

“The cases we are seeing are the younger groups that probably aren’t eligible for vaccines just yet,” Davis said.

More than 73 percent of people over age 65 has received at least one dose of the Covid-19 vaccine, and nearly half are fully vaccinated. Monday, the CDC reported that the Pfizer-BioNTech and Moderna vaccines are 90 percent effective against Covid-19 in the real world.

The increase in immunity among older adults is illustrated in the shift in age groups most likely to seek care for Covid-19.

Nationwide, “the number of 25-to-49-year-olds visiting U.S. emergency departments for diagnosed Covid-19 is now higher than the number of visits among patients 65 and older,” the CDC said in a statement to NBC News.

In Michigan, where cases have more than doubled in the past two weeks, doctors are seeing increases in patients between the ages of 10 and 60, with hospitalizations among people in their 50s rising at the fastest rate, said Dr. Joneigh Khaldun, the state’s chief medical executive.

“Even if you may be at lower risk for getting the virus, you can still get the virus,” Khaldun said. “People simply cannot let their guard down now. No one lives in a bubble.”

Dr. Usamah Mossallam, an emergency department physician at Henry Ford Health System in Detroit, cautioned that even younger people can get quite ill from Covid-19.

“I’ve also seen my fair share of people who require hospitalization and ICU admissions, and they’re in their 40s,” Mossallam said.

At Northwell health system in New York, Davis said that many of the Covid-19 cases are due to domestic travel.

“They went to a gathering somewhere out of state, whether it was family they hadn’t seen in a long time, or maybe a wedding,” Davis said. “Sometimes multiple family members come in from the same event.”

Spring break may also be partly to blame for the recent rise in cases, particularly as some states have rolled back restrictions and mask mandates, said Dr. Joseph Khabbaza, a pulmonary and critical care medicine specialist at the Cleveland Clinic. But vaccinations among the highest-risk demographics may prevent hospitalizations from rising as precipitously this time as they did during previous surges, he said.

“A lot of places opened up and we saw more travel for spring break, so we certainly expected a bump in cases,” Khabbaza said. “But when these people go back home to their workplace or to their loves ones and family members, at least more of the higher risk people are going to be vaccinated.”

The trend toward younger patients with milder illness is likely the reason many intensive care units are no longer inundated with severe cases.

At the pandemic’s peak last year, Dr. Todd Rice, director of Vanderbilt University Medical Center’s medical intensive care unit in Nashville, Tennessee, said he had 60 Covid-19 patients at once. Now, he has five.

The shift is likely because “we’ve changed the demographics of the people who are being affected” by the illness, Rice said.

The virus itself, however, is shifting and changing. “We know that about 26 percent of all sequenced virus is now the B.1.1.7” variant, Walensky said during Monday’s briefing. That variant has been shown to be more contagious.

“This variant is probably less forgiving, and more infections will occur,” Walensky said.

Khabbaza said that while he remains hopeful that a new wave of infections won’t be accompanied by as many hospitalizations and deaths, it’s hard to predict the full impact of the variants.

“We haven’t had a surge with a variant yet, so we’re not really sure what to expect for these next couple weeks and months,” he said.

Even if cases are milder, they can still result in debilitating “long-hauler” symptoms.

The possibility of long-term illness combined with more transmissible variants are major reasons to get vaccinated, even as cases are trending toward younger, healthier patients.

“The good news with the variants is, they are all mostly responsive to vaccinations,” said Dr. James McDeavitt, senior vice president and dean of clinical affairs at Baylor College of Medicine in Houston.

“We can still vaccinate our way out of this crisis.”

Even as millions of Americans flock to be vaccinated against COVID-19, there’s a pressing question on the flip side of these lines and hard-to-find appointments — how many people will decide, ultimately, not to get a vaccine and what’s that going to cost us?

Researchers with Imperial College London (ICL)’s COVID-19 Response Team recently took a stab at modeling what the future could look like with vaccine hesitancy in the mix — and it’s not pretty.

Even with highly effective vaccines against SARS-CoV-2, current levels of hesitancy in the United States could require the continuation of nonpharmaceutical interventions — such as closing workplaces and schools and wearing masks — through at least the end of 2022 to keep the pandemic under control.

The model also projected that thousands more people — both vaccinated and unvaccinated — could die and be hospitalized over the coming months because some people remain wary of the shots.

In the United Kingdom, where hesitancy is low, more than 80% of people recently surveyed about their willingness to get a vaccine said they had already been vaccinated or planned on it. In Germany, those who say they probably or definitely would get vaccinated is up to nearly 70%. In France, around half of people have said they would get a COVID-19 vaccine. The ICL team modeled these countries as “low,” “medium,” and “high” hesitancy scenarios, respectively, and they compared those results with an ideal scenario — 98% of people 15 and older are vaccinated.

The ideal scenario in the model assumes that a few people can’t take the vaccines because of allergies or preexisting health problems, but that everyone wants to get the vaccine.

Model Predicts Winter Surges

The model predicts several future surges in COVID-19 cases — mostly over the winter months — continuing into 2024, as people resume travel and return to offices and schools, restaurants, and entertainment venues.

Even in a scenario like that in the UK, which has highly effective vaccines and hesitancy is low — perhaps between 10% and 20% — the model shows that daily deaths will be nearly 9 times higher at the peak of the first surge because people opt out on vaccines. Those deaths will be concentrated among people who choose not to get the shots, but they will also affect those who are vaccinated because even the best vaccines are not 100% effective.

Over the next 2 years — through the end of 2022 — the model predicts about 30% more COVID-19 deaths in a low-hesitancy country like the UK, about 350% more deaths in a medium-hesitancy country like Germany, and about 770% more deaths in a country with high hesitancy like France.

Where does the United States fall in that mix? The ICL team didn’t model the US specifically, but recent surveys suggest that America is most closely aligned with Germany in the “medium” hesitancy position.

With medium hesitancy, the model showed 1744 deaths from COVID-19 per million people — with about 168 per million of those deaths in vaccinated people and 1576 in unvaccinated people, through the end of 2022.

Tricky to Model

There are some things to keep in mind about this model, though, said Rupali Limaye, PhD, director of behavioral and implementation science at the Johns Hopkins Bloomberg School of Public Health, Baltimore, Maryland.

She says hesitancy is hard to model because it clusters. Whether or not someone is likely to get the vaccine depends on a slew of factors, including what their friends post on Facebook, their race, age, gender, political affiliation, religion, and even their level of education.

“People that tend not to get vaccines tend to hang out with each other,” she said.

For the purposes of the model, the ICL team only factored age into the hesitancy equation. They assumed that in every other way, people were equal.

Limaye said the numbers in the ICL model look reasonable, but in real life, the deaths and hospitalizations as a result of hesitancy won’t be evenly spread. Some communities will be hit much harder by hesitancy than others.

Just knowing the numbers doesn’t help you intervene where it’s most needed.

Limaye, who wasn’t involved in the ICL modeling, has been working with Johns Hopkins colleague Shaun Truelove, PhD, to try to make similar estimates for the US. 

Truelove, a data modeler, said the ICL report painted a picture that was perhaps too pessimistic. 

First, he said, vaccine hesitancy has declined in the US and other countries.

“More and more people are getting vaccinated. We’ve got more data on safety, more data on effectiveness; and we also, at the same time, have had this re-emergence of the pandemic with the variants,” Truelove said. “Because of all that, willingness also goes up.”

He did point out that willingness could drop if people lose confidence in some of the newer vaccines, like AstraZeneca’s.

The model looks forward almost 2 years. Truelove said that’s a long time when you’re talking about such a fluid situation. When he’s making models, he doesn’t like to go any farther out than about 6 months.

Also, he said, models aren’t forecasts, but they can be useful to help scale potential impacts and help guide public policy decisions.

The study authors say they wholeheartedly agree on that point.

“Getting vaccinated is an individual choice; however, this choice has social consequences,” said Daniela Olivera Mesa, a PhD student at Imperial College London, in a news release. “Our work demonstrated that vaccine hesitancy can have a substantial health impact that affects both the vaccinated and unvaccinated populations. Building trust in vaccines is an important public health priority to control COVID-19.”
